Community Care Services Program (CCSP)
The Community Care Services Program (CCSP) is designed by Medicaid to provide Personal Support Services (PSS) to individuals needing assistance to carry out their Activities of Daily Living (ADL’s) due to an illness, injury or old age. These services provide members with the opportunity to stay in community and avoid nursing home placement. The intent of the service is to provide quality, competent care to CCSP members in the form of:
- Personal Care
- Housekeeping
- Home management
- Proper nutrition
- Medically-related activities
- Ambulation
- Respite care to caregivers
Affordable Medical Resources, Inc. is a Private Home Care Provider licensed by Medicaid to provide Personal Support Services under the Community Care Services Program (CCSP). At Affordable Medical Resources, Inc. we work with our clients or their families to develop personally tailored services to best serve your needs. Our goal is to provide services that maintain or increase the functioning capacity of the clients being served and focus on the relationship between the clients and their needs.
